Message from the superintendent: Vaccine appointments available for Freeport High School Students

Dear Freeport High School Families,

On Saturday, April 24th, Freeport Public Schools is partnering with Mount Sinai South Nassau to administer the COVID-19 vaccine to individuals aged 16, 17 and 18 only. The first dose of the vaccine will be offered on Saturday, April 24th at 519 Merrick Road in Rockville Centre. The vaccine pod is located on the corner of Merrick Road and Long Beach Road (across from the Pantry Diner).

Please use the link below to register for your first dose appointment. You must be available for your second vaccine dose on Saturday, May 15th .

  • Please bring proof of eligibility to your appointment (identification showing date of birth)
  • Minors (less than 18 years of age) should be accompanied by a parent /guardian
  • Individuals who do not meet these criteria and individuals who do not provide proof of eligibility will not be vaccinated

Please read the information below carefully regarding your appointment:

  • The vaccine pod is at 519 Merrick Road in Rockville Centre- corner of Merrick Road and Long Beach Road (across from the Pantry Diner)

  • Masks are required to enter the building

  • Please wear short sleeves

  • When you arrive, please pull into the parking lot. You will be approached by a parking attendant. Inform the parking attendant of your appointment time. Wait in your car and the parking attendant will bring you into the building when it is your turn to be vaccinated

  • Individuals who had any other vaccinations within 14 days will not be vaccinated

  • There are no requests for a specific vaccine

  • You will be asked to complete a NYS DOH attestation to confirm that you are eligible for the vaccine.
