District Information for Start of School - Packets and Forms

Dear Freeport Families,

This year rather than mailing the packet of district information, we are sending all the documents via email and posting it on the website. All the response forms can be filled out via a Google link. There is a page in the Information packet with all the links and QR codes. Below is a list of what you are receiving.

- Information Packet: this includes the following for your information:

  • Letter about Assessments, Attendance Policy, and Internet Acceptable Use Policy
  • Community Access Sheet (Important District contact numbers)
  • Instructions for accessing the Parent Portal
  • Information about the Freeport Public Schools Mobile App
  • E-mail notification option regarding sex offenders
  • Emergency Planning Guide for Parents
  • Contact information for the Office of Family and Child Services
  • How to sign up for our Mobile App
  • How to opt in for text messages
  • Google Links Page for forms to be filled out and returned.

- Code of Conduct Summaries for Elementary and Secondary Students

- Child Nutrition Booklet - this contains the Household Eligibility form that must be filled out for every household. The attached document is a fillable form that you can download and email to your child's school teacher. You will only need one per family.

The following materials must be signed by the parent/guardian. You can do this digitally through the QR Code/link or print and return it to your child's school on the first day.

  • Code of Conduct and Internet Acceptable Use Sign Off forms
  • Media Release Form - only if you do NOT want your child's name or photo to be released to the media if an occasion arises for media coverage of school events
  • Census Form (Return just ONE census form for the entire family)

These documents are attached, and are also available on our website. If you prefer, copies of the documents are available at each school building.

If you have any questions, please contact your building principal. You will receive building specific information from your child's school buildings in the mail. We hope you enjoy the rest of the summer and look forward to seeing our students on the first day of school, September 1st.
