Important - Please Take the Digital Equity Survey

Dear Freeport Families,

The Freeport School District and New York State recognize that students' home access to the internet has become fundamental for schools across the nation as they plan strategies for continuous learning. In order to better serve our families, students and educators, it is extremely important to have an accurate understanding of students' access to the internet as well as the type of devices they use for homework and school related instruction. To gather this information, New York State is requesting that all parents/guardians complete the quick survey for children (grades K-12) on or before November 19, 2021. The survey consists of nine questions regarding the types of devices, internet access and whether the device was school or personally provided.

We are requesting your cooperation in completing the New York State requiredDigital Equity Survey by scanning the QR code below or by typing the internet address located within this letter in your device's internet browser.

If there are multiple K - 12 children in your home, you will need to fill out a form online and submit a response for each child. We are required by the state to collect this information for each student, as each student's technology access may be different, even within the same household. The link to this surveycan also be found on our District website and below.

Important: You will need your child's Student ID Number in order to fill out the Digital Equity Survey. You will be receiving a letter from your child's school with their ID number.
Additionally, you can find the ID number in the Parent Portal. The student ID is also the number in your child's Google email account log-in.
